After days of flashing and frustration, it turns out the culprit was the proximity sensor that was stuck on "near". Another day of researched later I put all this together, figured I'll share it.
1. Try using compressed air in 3.5 mm headphone jack of your phone to remove particles that may be stuck to the proximity sensor and causing the false positive. I used a vacuum cleaner and it WORKED!
2. If that does not help follow this tutorial to reset the proximity sensor http://ift.tt/1gNHBeZ
3. This is a video on how you can get to the proximity sensor of your S4 and and clean it or replace it http://ift.tt/1C5c6KN
Hope that saves someone the headaches.
